Parents and guardians can use this form to give permission for a student to attend a FACES field trip.

A separate signed waiver is required for each student before the student may attend the field trip.

Permission to Attend: I give permission for my child named above to participate in the field trip listed on this form.

Transportation Permission: I authorize FACES staff, approved volunteers, or contracted transportation providers to transport my child for this field trip as needed.

Emergency Medical Care: If I cannot be reached in an emergency, I authorize FACES staff to seek emergency medical treatment for my child. I understand that I am responsible for medical costs not covered by insurance.

Release of Liability: I understand that field trips may involve normal risks related to travel, walking, public places, and group activities. I agree not to hold FACES, its staff, volunteers, officers, or partner locations liable for injury, loss, or damage except where prohibited by law.

I am the parent or legal guardian and give permission for this student to attend the field trip. I authorize emergency medical care if needed and understand that FACES will make reasonable efforts to contact me first. I have read and agree to the waiver and release language above.
